Virginia State Health Requirements for Entrance to School
● All Required Health paperwork must be submitted to the School Clinic by August 1st.

New Students entering, Kindergarten, or transferring to St.Theresa Catholic School are required to have a completed VA School Entrance Health Form: this includes a Comprehensive Physical Examination Report and a Current Immunization Record. These must be Dated within ONE calendar year of the first day of school.
Returning Students: Each School Year must submit a Confidential Health History Form and (if applicable) an updated immunization record. Rising Seventh Grade Students ● Tdap: A booster dose of Tdap vaccine (Tetanus, Diphtheria, and Pertussis) is required for students entering the 7th Grade.
● Meningococcal: Effective July 1, 2021, a minimum of 2 doses of MenACWY vaccine. The first dose should be administered prior to entering 7th grade. The final dose should be administered prior to entering 12th grade.
● Human Papillomavirus Vaccine (HPV): Effective July 1, 2021, a complete series of 2 doses of HPV vaccine is required for students entering the 7th grade. The first dose shall be administered before the child enters the 7th grade. After reviewing educational materials approved by the Board of Health, the parent or guardian, at the parent's or guardian's sole discretion, may elect for the child not to receive the HPV vaccine. Exemptions Parents who choose not to have their child immunized, must complete a Certificate of Religious Exemption Form. This form must be notarized.
